Forms

These are common forms used for administrative requests, or information sources.  

Undergraduate Business Petition  - Used to request exception to College of Business requirements or rules (not for course equivalencies). 

Petition for Late Withdrawal - Used to request to drop a class after the deadline, in combination with the Instructor's Memorandum , which must be completed by the professor of each course.

CLAST Exemption Form  -  Used when a student has attained exemption from either the math or English portions of the CLAST exam by alternative means.  Complete and submit to a Business Undergraduate Studies office.  

Course Equivalent/Substitution Petition  - Used to request a transfer course be considered a  - Used to request a transfer course be considered as a substitute or equivalent to an FAU course, if not accepted on the student's initial transfer program sheet.  This form is not to be submitted prior to enrolling at FAU.

Graduation Audit Request  - Used to request a formal graduation audit by an academic advisor.  This form will only be processed if a student lacks 11 or fewer courses for graduation (including in-progress classes).

Change of Major - Used to change from one College of Business major to another College of Business major.  Not to be used to change Colleges.  Complete offline and submit to a Business Undergraduate Studies office.

International Business Certificate Request Form - Used to request a Certificate of International Business (usually filed in your gradation semester).

The listing of Major Programs provides required course numbers and titles for each College of Business Major, as a guide for new or transfer students.  Students may request a graduation audit on facts.org  to check progress toward a degree.  The college strongly suggests you see an academic advisor to assess your degree progress.

Transient Student Policy/Form for Graduating Seniors  - The College of Business policy does not allow students to take transient courses in their graduating semester.  This is because the transfer credit cannot usually be processed in time to certify degrees.  This form can be used for a student to self-waive this policy, but assume all associated risks.


University Forms

Application for Admission to FAU  - Used to apply to FAU's undergraduate degree programs.

Degree Application - Used to apply for your degree (due no later than the 3rd week of your graduating semester).

Enrollment Verification Form   - This is a Registrar form, allowing you to obtain documentation from the Registrar's office concerning your enrollment status at FAU.

Forgiveness Policy Form  - FAU form that allows students to "forgive" prior grade on a repeated course. 

Gordon Rule Evaluation Form  - Used by the Registrar's office, to apply transfer work toward the state's Gordon Rule requirements.

Immunization Form  - This Student Health form is for the required proof of immunization for all new FAU students.

Transient Student Form  (use Transfer Services link)  - Used for taking courses as a visiting student at other Florida universities and community colleges.   This link is to the electronic version, allowing you to submit the form by web, and eliminating the need to hand-carry the form.  The electronic form is usually processed in 1-2 working days.  Older paper versions are available from the Undergraduate Studies or Registrar's offices, if needed.
